We are growing and seeking qualified Licensed Clinical/Independent Social Worker to service our patients across the state of Georgia.

Licensed Independent Social Worker – Clinical Practice (LISW-CP) play a vital role as part of the care team within our organization. This role provides patients with the essential psychosocial, psychotherapy, and counseling support necessary to cope with chronic, acute, or terminal illnesses.

LISW/LCSW perform patients visits via telehealth remotely from their home. This is a full-time salary-based 8‑hour shift (8:00am‑5:00pm; Monday‑Friday) position.

What Are We Looking For?

Your Health is currently looking for a Social Worker – LCSW to join our growing primary care family. A successful Licensed Social Worker will be able to perform these essential duties and responsibilities. Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient’s needs, strengths, and challenges.
  • Screen for mental health conditions, substance abuse issues, domestic violence, and other social determinants of health.
  • Provide individual, couples, family, and group counseling or psychotherapy for depression, anxiety, grief, trauma, and adjustment to illness or life changes.
  • Offer immediate support and intervention for patients experiencing acute mental health crises, including suicidal ideation, self‑harm, or emotional distress.
  • Develop and implement cognitive assessment care plans tailored to each patient’s unique needs, integrating social determinants of health and behavioral health.
  • Accurately document and log care management, collaborating with the care team to keep patient care plans up to date.
  • Facilitate integration of behavioral health services into primary care settings, ensuring patients have access to appropriate mental health and substance abuse resources.
  • Facilitate communication and coordinate among the patient’s care team members to ensure a comprehensive integrated approach to health care delivery.
  • Communicate with providers and staff via phone, email, Teams, Athena, or in‑person.
  • Collaborate closely with care team members to coordinate comprehensive care plans addressing both medical and psychosocial needs.
  • Educate patients and families about mental health conditions, treatment options, coping strategies, and community resources.
  • Advocate for patients to ensure their needs are addressed within the healthcare system and community settings.
  • Assist with access to social services, disability benefits, housing assistance, or legal support.
  • Maintain accurate and confidential documentation of assessments, treatment plans, progress notes, and communication in accordance with legal and ethical standards.
  • Recognize and respect the cultural, ethnic, and socioeconomic diversity of patients, tailoring interventions to meet unique needs and preferences.
  • Interact with patients via email, patient portal, telephone, text, in‑person, etc. to gather information, schedule and confirm appointments, and provide pre‑appointment instructions.
  • Respond to patient inquiries, resolve scheduling conflicts, and assist in rescheduling appointments when necessary.
  • Provide exceptional customer service, exhibiting a compassionate and empathetic attitude while addressing concerns and complaints professionally and promptly.
  • Collaborate with internal and external resources to facilitate seamless operations.
  • Perform general administrative tasks and support other staff as needed.
  • Utilize the company’s software systems to enhance patient care and staff efficiency.
Qualifications
  • Must be a licensed Clinical/Independent Social Worker.
  • Master’s Degree with appropriate licensure for LMSW or LISW‑CP. License must be in good standing.
  • Experience or certification as a substance use disorder (SUD) specialist is preferred.
  • Minimum of one year of experience in a healthcare setting is preferred.
  • Minimum of one year of experience in counseling preferred.
  • Proficient in utilizing technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Ability to read and communicate effectively.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Ability to manage and demonstrate effective leadership skills.
  • Good interpersonal and communication skills under all conditions and circumstances.
  • Ability to foster a cooperative work environment.
  • Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices or facilities; must be a licensed driver with an insured automobile in good working order.
