You will conduct psychosocial assessments, develop individualized plan of care, and document in the EMR to support patient care.

The role requires GA licensure; strong communication, problem solving and collaboration, with occasional travel to facilities as part of a fast-paced office environment.

Competenze

• Master's degree in social services, psychology, or closely related field.
• Georgia licensure as a professional counselor (LPC).
• Current CPR certification.

Mansioni

• Conduct psychosocial assessments and develop treatment plans.
• Coordinate care with MDs, NPs, and PAs and other staff.
• Maintain accurate EMR documentation and patient records.
• Provide short-term counseling and crisis intervention as needed.
• Educate patients about resources, insurance, and disease management.
