Qualifications

• Master of Social Work degree.
• Six months experience in healthcare delivery system.
• Current license as a Licensed Master Social Worker (LMSW).

Responsibilities

• Provide comprehensive psychosocial assessments for patients and families.
• Create a therapeutic framework in counseling to support problem solving with patients and families.
• Collaborate with Care Coordinator on transition of care strategies in complex cases.
• Identify community resources and link patients and families to services.
• Communicate progress with the interdisciplinary team and revise plans as needed.
• Evaluate patient/family progress and participate in care conferences and rounds.
• Integrate medical and nursing information in assessments.
• Ensure reporting of suspected abuse/neglect and address domestic violence issues.
• Document interventions in the patient’s medical record.
• Participate in process improvement activities in the care management department.
