EverCare Mobile Health is seeking a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Professional Counselor (LCPC) to join our in-person team in Idaho Falls. The role focuses on delivering psychotherapy within long-term care settings and collaborating with care teams to improve resident well-being.

• Active LCSW License in Idaho.
• Must maintain licensure and remain in good standing throughout employment.
• Ability to pass an enhanced background check through the Department of Health and Welfare.

Responsabilidades

• Provide psychotherapy to individuals experiencing psychological, emotional, or behavioral challenges.
• Oversee LPC providers and supervise CBRS workers as needed.
• Facilitate therapy groups and complete necessary assessments (e.g., CDAs) for insurance compliance.
• Develop and implement individualized treatment plans and maintain up-to-date documentation in the agency’s EHR system.
• Collaborate with interdisciplinary teams, including facility staff, PMHNPs, PCPs, and other behavioral health professionals.
