Ambercare Hospice in Española, NM, is seeking a licensed social worker (LMSW or LCSW) to join a refreshed hospice team. You will assess patients and families, coordinate care plans, and provide counseling and resource assistance in a compassionate setting.

The role offers a $5,000 sign-on bonus, relocation assistance, and a supportive environment with manageable caseloads and comprehensive benefits. A valid NM driver’s license is required.

Qualificações

• Master's Degree in Social Work required (NM).
• Current State Licensure in Social Work (LMSW or LCSW) (NM).
• Minimum of one year's experience in health care or hospice setting.
• Valid driver's license and proof of insurance required.

Responsabilidades

• Performs psychosocial assessment of patients and families/caregivers in the hospice program and updates care plans accordingly.
• Provides compassionate support to patients and families, coordinating with the care team.
• Counsels patients and families on coping with stress and related challenges.
• Documents patient services accurately and timely to reflect the plan of care.
• Assists with applications for SSI, Hospice, Medicaid, and waivers, connecting to community resources.
• Serves as liaison between patients, families, and community agencies.

What You'll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies
