Job summary

Ambercare Hospice is seeking a compassionate LCSW or LMSW to join our Espanola, NM team. This role offers a $5,000 sign-on bonus and relocation assistance for qualified candidates, with a focus on delivering compassionate psychosocial care to patients and families within a refreshed team environment.

Responsibilities include psychosocial assessments, care plan collaboration, and resource coordination, along with comprehensive benefits and a supportive work culture in Northern New Mexico.

Qualifications

• Master's degree in Social Work is required.
• Current NM LMSW or LCSW licensure is required.
• Minimum of one year of health care or hospice experience.
• Valid driver’s license and proof of insurance.

Responsibilities

• Perform psychosocial assessment of patients and families/caregivers in the hospice program and address identified needs in the plan of care.
• Provide exceptional service to patients, families, referral sources, coworkers and other customers.
• Counsel patient and family caregivers and staff regarding stress and coping difficulties.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assist with applications for SSI, Hospice, Medicaid and Medicaid Waiver; obtain resources from community agencies.
• Act as liaison between patients, families and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

• Social Work Degree
  • New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work
  • New Mexico: LMSW or LCSW
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
