Job summary

Nexum HC is seeking a licensed mental health professional to provide psychotherapy onsite at a skilled nursing and rehabilitation facility in Oak Park, Illinois.

You will work with older adults facing depression, anxiety, grief, adjustment concerns, and other behavioral health needs while collaborating with nursing staff, physicians, social services, and the facility’s interdisciplinary team. This is a full-time role with about 25–30 direct hours per week, plus documentation time and admin tasks.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or a related field.
• Active Illinois license as an LCSW, LCPC, or LMFT.
• Strong clinical assessment, treatment planning, documentation, and communication skills.
• Ability to work independently onsite while remaining connected to a supportive clinical team.
• IM+CANS certification preferred.

Responsibilities

• Complete clinical assessments and diagnostic evaluations.
• Develop individualized, evidence-based treatment plans.
• Provide individual psychotherapy and facilitate group therapy as appropriate.
• Maintain accurate and timely clinical documentation in our EHR.
• Coordinate care with facility staff and the interdisciplinary treatment team.
