Job summary

Family First Health is seeking an independently licensed LCSW, LPC, or LMFT to join our Lebanon team as a Behavioral Health Consultant. You will work within an integrated primary care team, providing brief assessments and evidence-based interventions alongside medical providers to address patients' behavioral health needs.

The role emphasizes collaborative care, development of patient-centered plans, and coordination with specialty services when higher levels of care are needed.

Qualifications

• Current, unrestricted PA license as LCSW, LPC, or LMFT.
• Experience in integrated primary care or hospital settings preferred.
• Ability to collaborate with multidisciplinary care teams.

Responsibilities

• Provide brief behavioral health assessment and evidence-based interventions in primary care.
• Collaborate with Primary Care Providers and interdisciplinary team members.
• Develop patient-centered treatment and care plans.
• Use screening and measurement tools to monitor progress.
• Provide consultation and behavioral health education to patients and staff.
• Utilize Motivational Interviewing to support behavioral change.
• Assess safety concerns and coordinate follow-up care.
• Connect patients with specialty behavioral health and community resources.
• Document care in the electronic health record and communicate with the team.
