Job summary

The role requires a Master’s in Social Work and a Florida LCSW/LSW license, with 2+ years of relevant experience.
Qualifications
- Doctorate in Social Work from NASW-accredited university.
- Master’s degree in Social Work required.
- Florida Licensure as LCSW or LSW is required.
- Experience in clinical social work with case management is preferred.
Responsibilities
- Performs psychosocial assessments and provides psychotherapy to patients and families coping with medical issues.
- Maintains comprehensive resources and networks to assist with post-acute transitions.
- Collaborates with the multi-disciplinary team to document psychosocial determinants for safe discharges.
- Provides counseling services, crisis intervention, stress management, and bereavement support.
- Participates in staff education and coordination of post-acute care planning.
