AdventHealth in Kissimmee, FL is seeking a qualified licensed social worker to provide psychosocial assessments and psychotherapy for patients and families facing medical issues. You will collaborate with a multidisciplinary team to document psychosocial determinants and support safe discharges, while coordinating resources for post-acute care and bereavement support.
The role requires a Master’s in Social Work and a Florida LCSW/LSW license, with 2+ years of relevant experience.

Performs psychosocial assessments and provides psychotherapy to patients and families coping with medical issues. Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community. Collaborates with the multi-disciplinary team to establish and document psychosocial determinants for safe discharges. Serves as a consultant and educator to the multi-disciplinary team regarding the psychosocial components of illness, disability, and hospitalization. Facilitates staff emotional debriefing and ongoing support groups. Applies knowledge of local, state, and federal programs to assist with appropriate referrals and coordination of post-acute care. Participates in the development of programs to support patients and families dealing with complicated medical issues. Attends physician rounds and forums to facilitate communication and clinical social work input to the interdisciplinary team. Demonstrates working knowledge of the involuntary commitment process. Provides counseling services, crisis intervention, stress management, and bereavement support. Other duties as assigned.
