Job summary

Mobile Medical in Jacksonville, FL is seeking a compassionate Licensed Clinical Social Worker (LCSW) to provide behavioral health support, counseling, care coordination, and resource management to patients throughout Duval County.

You will build meaningful relationships with patients, addressing social and emotional barriers that impact health and quality of life, while collaborating with care teams to improve outcomes. Flexible schedule with telehealth and field visits.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Active Florida Licensed Clinical Social Worker (LCSW) license.
• Minimum of 1 year of clinical counseling experience.
• Valid Florida Driver’s License and reliable transportation.
• Strong communication and relationship-building skills.

Responsibilities

• Provide counseling, psychosocial assessments, care coordination, and community resource support.
• Collaborate with providers, caregivers, and interdisciplinary teams to improve patient outcomes.
• Maintain accurate documentation and ensure compliance with Medicare and company standards.
