Job summary

University of Pennsylvania in Philadelphia seeks a Licensed Clinical Social Worker to join a collaborative care team in primary care for patients with mental health and substance use disorders.

The LCSW will support care coordination with medical providers and specialists, administer evidence-based psychosocial treatments, and document progress while meeting regulatory standards. The role requires an LCSW with PA licensure and experience in behavioral health.

Qualifications

• Master’s degree in social work, nursing, counseling or psychology and 2–3 years of experience required.
• LCSW in Pennsylvania preferred; 3+ years experience in behavioral health care industry, assessment and treatment planning for common mental health, experience with evidence based counseling techniques preferred.
• Experience providing crisis and/or utilization management services; 3+ years of clinical experience working with patients with substance use disorders preferred.
• Experience working in safety net health clinics or low barrier models for patients with addiction preferred.

Responsibilities

• Assess patients for common mental health and substance use disorders using structured behavioral health assessment and monitoring tools.
• Provide behavioral interventions using evidence-based techniques such as motivational interviewing, problem-solving treatment, behavioral activation, relapse prevention, or other treatments as appropriate.
• Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
• Participate in caseload consultation with the psychiatric and/or addiction medicine consultant and communicate treatment recommendations to the medical provider.
• Document patient progress and treatment recommendations in EHR and ensure regulatory compliance.
