Transformcap is seeking an LCSW Clinical Specialist II for Synchronous AHPC Partnership (CA) - Remote. The role advances a world-class clinical experience for Participants, collaborating with the Health Care Industry and clinical teams to improve quality of life and wellbeing.

You will support participants and teams, facilitating sessions, note-taking, and care planning while staying current on clinical care advances and tele-health practices. This is a fully remote, impactful clinical role.

Formación

• Licensed LCSW with ability to practice without supervision.
• Experience with tele-health platforms is preferred.
• Comfortable with technology and virtual sessions.
• Ability to work remotely from a private location.

Overview

LCSW Clinical Specialist II for Synchronous AHPC Partnership (CA) - Remote

The Clinical Specialist II is an active member of the clinical team and is responsible for advancing a world-class clinical experience. This role supports Participants, Clinical Specialists, and the Health Care Industry, delivering high-quality care to help Participants reach agreed-upon goals for improved quality of life and wellbeing. The Specialist partners with Operations to improve process efficiency and access, and with the Clinical team to enhance clinical care and support.

Key Responsibilities

• Productivity standards may vary by customer contract; however, the general standards include:

  • 30-37 hours of direct counseling care per week

  • Collaborative note-taking during sessions

• Treatment plans and other documentation as required

• Treatment team meetings with the customer, as applicable

• Weekly Synchronous Health team meeting

• Advocates for Participants in decisions regarding meeting their clinical needs

• Liaises closely with Customer Success activities and requests, advocating for the customer’s needs to be met

• Brings successful models and techniques from other experiences and bodies of evidence to best support participants

• Supports the development of materials for communication, outreach, and marketing of Synchronous Health’s services to participants, clinicians, partners, and within the industry (e.g., conference presentations, networking events, professional organizations, etc.)

• Serve as the first point of contact for escalation for participants who are displeased with any aspect of their experience

• Reports to the Operations manager as the second point of escalation if participants’ concerns are unresolved

• Keep abreast of new developments in clinical care, including analog and digital techniques

• Other duties as assigned

Requirements

• Licensed LCSW (license must be able to practice without supervision)

• Experience working with children is preferred, not required

• Informed on state tele-health policies

• Comfortable with technology and conducting sessions via tele-health

• Capability to build a therapeutic relationship in a virtual platform

• Polished, professional virtual presence

• Strong ability to identify markers for the appropriate level of care to meet patient needs

• Experience with treating a variety of conditions and disorders as part of a comprehensive treatment plan with an interdisciplinary team

• Ability to use technologies as part of practice for administrative and clinical support (scheduling, notes, chat, web-based or app-based interventions)

• Ability to work remotely in a secure, private location (outpatient office or home office preferred; coffee shops are not acceptable)

• Access to a smartphone, a laptop, and an internet connection

• Must pass a background check

• Bilingual a plus
