Job summary

Required are 2–4 years of LCSW experience in managed care and a MSW; roles include coordinating services, educating families, and supporting advance care planning and end-of-life care discussions.

Qualifications

• LCSW with 2–4 years social work experience in a managed care environment.
• MSW fieldwork not included.

Responsibilities

• Coordinate services such as Home Health, Hospice, or other levels of care.
• Educate patients and families on adapting to diagnosis, treatment, and life changes.
• Participate in multi-disciplinary teams and assist with advance care planning and POLST.
• Coordinate symptom management and end-of-life services.
• Assess and refer for Medi-Cal and SSI/SSDI as needed.
