Hill Physicians Medical Group is seeking a licensed clinical social worker to provide telephonic psychosocial assessments and interventions. You will assist patients and caregivers with care transitions, coping with illness, and related life situations, including emotional, mental, and substance abuse aspects.

The role involves coordinating services across care settings, education for families, and participation in multidisciplinary teams, with emphasis on advance care planning and eligibility

Formación

• LCSW with 2–4 years social work experience in the last 5 years.
• Managed care experience is preferred.

Responsabilidades

• Coordinate services such as Home Health, Hospice, SNF, or board and care.
• Educate patients and caregivers about adaptation to diagnosis, illness, and treatment.
• Participate in multi-disciplinary health care teams.
• Assist in advance care planning and POLST completion.
• Coordinate symptom management and end-of-life services.
• Assess and refer for Medi-Cal or other insurance eligibility and SSI/SSDI as needed.

We do it because it's right!Job Description:Within Hill Physicians’ care coordination framework, provide telephonic psychosocial assessments and interventions to assist patients and caregivers in making care transitions and coping with their illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.Job ResponsibilitiesCoordinating appropriate services, such as Home Health or Hospice care, appropriate other levels of care such as SNF, Board and Care, Care Givers at home.Education of patients, family members, and care givers around issues related to adaptation to the patient’s diagnosis, illness, treatment and or life situation.Participation in multi-disciplinary health care teamsAssist in advance care planning and completion of Advance Directives and POLST.
