Job summary

This hospice social worker position provides psycho-social assessments and interventions under the supervision of the medical team and the hospice interdisciplinary group, with a focus on patient wishes and comfort at end of life.

You will document assessments and visit notes in the EMR within 24 hours, coordinate with IDG and physicians, support advance directives, plan for bereavement, and participate in case conferences and referrals to community resources.

Qualifications

• Lic. Clin Social Work license required.
