Job summary

Lions Rehab Center in Cumberland, Maryland, is seeking a compassionate Social Worker (LCSW-C) to join our skilled nursing facility team. You will provide social work services focused on residents' physical, emotional, and social well-being, developing social histories and individualized care plans.

You will collaborate with administration and department heads, stay current with regulations, and assist in admissions processes, ensuring a supportive environment for residents and families.

Qualifications

• Bachelor's degree in Social Work or related field is required.
• Must possess any certifications/licensures as required by the State to practice in long-term care.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct social services programs of the facility.
• Develop, administer, and coordinate department policies and procedures.
• Keep abreast of federal and state regulations and make recommendations on changes in policies.
• Participate in community planning related to residents and services.
• Prepare necessary forms and reports and submit them as required.
• Provide written and oral reports to the Administrator and department heads.
• Orient residents and families to the Nursing Center and assist in adjustment.
• Develop social histories and assessments with care plans.
