Job summary

Deer Oaks Behavioral Health in Mount Pleasant, TX is seeking a Licensed Clinical Social Worker (LCSW) to support long-term care facilities with therapy, assessments, and care planning onsite, Monday through Friday.

Join a multidisciplinary team, document using the Geriatric Notes Tool, follow HIPAA guidelines, and build strong relationships with facility staff while delivering evidence-based geriatric care. This role offers autonomy with strong administrative support.

Qualifications

• Active, unrestricted LCSW license.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate treatment with nursing, social services, rehabilitation, and medical providers.
• Maintain complete and timely documentation using the GNT and ensure HIPAA compliance.
