Job summary

Deer Oaks Behavioral Health invites a licensed clinical social worker to join our clinical team, providing therapy and assessments onsite in long-term care facilities. You will participate in care planning, collaborate with interdisciplinary staff, and ensure HIPAA-compliant documentation to support residents’ emotional and behavioral well-being.

The role offers Monday–Friday schedule, autonomy, and strong administrative support, with a focus on geriatric behavioral health and quality patient

Qualifications

• Active, unrestricted LCSW license required.
• Experience with geriatric behavioral health and long-term care settings preferred.
• Ability to work independently within an interdisciplinary team.

Responsibilities

• Provide individual, family, and group therapy for LTC residents.
• Conduct psychosocial assessments and treatment planning.
• Coordinate care with nursing, rehabilitation, and medical staff.
• Maintain timely and compliant clinical documentation.
