Job summary

Deer Oaks is seeking a Licensed Clinical Social Worker (LCSW) to work onsite in one or more long-term care facilities within your local community. This part-time role involves therapy, assessments, care planning and behavioral health support, with strong clinical and administrative backing.

You will join an integrated team delivering geriatric care, coordinating with nursing and other clinicians, and ensuring high-quality, confidential care for residents.

Qualifications

• Active, unrestricted LCSW license.
• Strong written, verbal, and interpersonal communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and behavioral issues in LTC settings.
• Deliver behavioral management and goal-oriented interventions aligned with geriatric care best practices.
• Collaborate with nursing, social services, rehabilitation and medical providers to coordinate treatment.
• Participate in care plan discussions, facility meetings and case collaborations.
• Maintain documentation using our GNT and ensure confidentiality and quality care.
