Job summary

CFG Health Systems, LLC is seeking a Licensed Clinical Social Worker to provide comprehensive mental health care for inmates within a correctional facility in New York. You will perform intakes, assessments, and crisis interventions, delivering individual and group counseling while developing tailored treatment plans in collaboration with the treatment team.

You will maintain documentation, participate in team-based care, and ensure safety and compliance with policies and regulatory

Qualifications

• Master’s degree in social work, psychology, counseling, or related field.
• Current and valid licensure as an MSW, LCSW, LMSW, LMHC, LMFT, LCAT or equivalent in NY state.

Responsibilities

• Conduct mental health intakes, assessments, and crisis interventions.
• Provide individual and group counseling tailored to inmate needs.
• Develop and monitor individualized treatment plans with the treatment team.
• Maintain accurate progress notes, treatment documentation, and discharge summaries.
• Coordinate care with the multidisciplinary team and refer to external resources on release.

Position SummaryThe Licensed Clinical Social Worker provides comprehensive mental health care and support to the inmate/patient population within a correctional facility.
