Job summary

Middleburg Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to join our team of clinicians in caring for residents in our long-term care setting. You will monitor, assist, and encourage patient progress, and participate in discharge planning as required.

COVID-19 vaccination is required, with accommodations considered per law. You will direct day-to-day nursing assistant activities, chart care, administer medications, and ensure narcotics control.

Qualifications

• Must hold a current and valid LPN license in the state employed.
• Long term care/post-acute care setting experience preferred.

Responsibilities

• Direct the day-to-day functions of Nursing Assistants per current regulations.
• Assist in planning the nursing services portion of residents' discharge plans.
• Complete accident/incident reports as necessary.
• Chart nurses’ notes reflecting care provided and resident response.
• Prepare and administer medications as ordered by physician, per regulations.
• Order prescribed meds, supplies, and equipment per policy.
• Ensure narcotics handling and records comply with regulations.
• Perform daily rounds to ensure nursing staff meet standards.
• Notify physicians and family when a resident's condition changes.
• Attend center in-service programs as required.
• Complete daily nursing evaluations and documentation in Point Click Care.

Responsibilities:

• Direct the day-to-day functions of the Nursing Assistants in accordance with current rules, regulations, and guidelines that govern the long-term care center
• Assist in planning the nursing services portion of the resident’s discharge plans as required
• Complete accident/incident reports as necessary
• Chart nurses’ notes in an informative and descriptive manner that reflect the care provided to the residents, as well as the residents’ responses to the care
• Prepare and administer medications as ordered by the physician in accordance with federal, state, and local regulations and as required by center policy
• Order prescribed medications, supplies, and equipment as necessary and in accordance with established policies
• Ensure that narcotics are handled in accordance with federal, state, and local regulations and as required by center policy and that narcotic records are accurate for your shift
• Make daily rounds of your unit/shift to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards
• Notify the residents’ attending physicians and next-of-kin when there is a change in the residents’ conditions or if the residents are involved in an incident or accident
• Attend and participate in all center in-service programs as required
• Daily Required Nursing evaluations and documentation is completed in Point Click Care.

Qualifications:
