Job summary

Ohana Pacific Health is seeking a compassionate Community Health Worker to support kūpuna on Hawai'i Island. The role involves in-person care, hospital and rehab support, and community visits with some telephonic contact based on patient needs.

You will help patients navigate care, assist with paperwork, and coordinate follow-up services, aiming to improve access to health resources and promote self-management of conditions.

Qualifications

• High school diploma or equivalent is required.
• Associate or Bachelor’s in social work or other healthcare field, CHW certificate, or nurse aide training preferred.
• Proficiency in basic computer skills including navigating web, EMR, and MS Office.
• Experience in healthcare or home health or geriatrics preferred.

Responsibilities

• Support in carrying out treatment goals or care plans created by APP.
• Assist patients in improving healthcare access and health knowledge.
• Coordinate referrals and arrange follow-up services.
• Collaborate with APP to develop patient wellness goals.
• Answer patient inquiries and assist with paperwork.
• Facilitate remote visits via telephone or video conferencing.
• Educate patients on care processes and reduce anxiety.
• Identify barriers to care and refer to resources.
