Job summary

Kalele Care Services, founded by Ohana Pacific Health, seeks a compassionate Community Health Worker for an on-site Kauai role. You will support kūpuna across the care continuum, with emphasis on transition of care, health education, and social services in hospital, rehab, and community settings.

In-person and telephonic services may be required based on patient needs. Strong empathy, communication, and teamwork with care providers are essential to improve access and outcomes for Hawaii’s aging

Qualifications

• High school diploma or equivalent.
• Associate or Bachelor’s degree in social work or other healthcare related field, CHW certificate, or nurse aide training preferred.
• Proficiency in basic computer skills including navigating EMR, computer systems and MS Office.
• Healthcare experience or work with geriatric population preferred.

Responsibilities

• Support care plans and goals created by APP in person or in community settings.
• Help patients improve access to care and health knowledge.
• Coordinate community referrals and arrange follow-up services.
• Collaborate with APP on wellness and self-management goals.
• Assist with patient inquiries and paperwork completion.
• Facilitate remote visits via telephone or video conferencing.
