Ohana Pacific Health is seeking a compassionate Community Health Worker for an on-site role on Maui. The position focuses on caring for kūpuna, supporting transitions of care, health education, and social services across the care continuum.

The role collaborates with APP, promotes access to care, and helps reduce barriers such as transportation and language needs while engaging families and communities in health resources.

Qualifications

• High school diploma or equivalent.
• Associate or Bachelor’s degree in social work or healthcare field, CHW certificate, or nurse aide training preferred.
• Proficiency with basic computer skills including navigating EMR systems and MS Office.

Responsabilités

• Carry out treatment goals or care plans created by APP.
• Assist patients in improving healthcare access and health knowledge.
• Coordinate with APP on developing patient treatment goals.
• Assist patients with paperwork and arrange for follow-up services.
• Facilitate remote visits between patient and provider via telephone or video.
• Educate patients on care processes and reduce anxiety and fear.
• Identify barriers to care and refer to additional resources as needed.
