This role focuses on pre-visit planning, record gathering, and assisting with annual wellness visits to ensure care elements are completed and documented accurately in the EHR. Korean language proficiency is required for effective patient communication.

Qualifications

Responsibilities

• Review upcoming schedules and records ahead of appointments.
• Conduct pre-visit chart review to flag preventive care gaps and missing records.
• Gather outside records, screening results, specialist reports, and labs.
• Support medication reconciliation and other patient prep.
• Ensure completed services and clinical information are properly captured in the EHR.
