Advanced Medical Management, Inc. is seeking a Care Navigator Coordinator in Long Beach, CA. You will serve as a trusted non-clinical advocate for Medicare Advantage patients, helping with language barriers, scheduling, transportation, and access to pharmacy and benefits.

The role requires Korean-English fluency for current Los Angeles / Orange County assignments, and you will work across PCP practices, pharmacies, and health plans to resolve patient needs with a high-touch approach.

Qualifications

• Minimum qualifications include a high school diploma or equivalent.
• Bilingual fluency required; Korean-English for LA/OC assignments.
• Two+ years in healthcare, care coordination, or patient navigation preferred.

Responsibilities

• Provide Korean-English language assistance for navigation, scheduling, pharmacy, and care coordination.
• Coordinate scheduling of specialist visits, imaging, tests, and other services; track referrals and appointments.
• Arrange transportation, identify barriers, and connect patients with community resources as needed.
• Document interactions and maintain organized follow-up with patients and PCP practices.
