Job summary

CARE FIRST HOME HEALTH CARE INC. seeks a diligent Intake & Scheduler to manage patient referrals and coordinate scheduling in a home health setting. You will work with the WellSky/Kinnser EMR to ensure accurate intake, obtain authorizations, and support seamless transitions into care services.

You will collaborate with intake, clinicians, billing, and referral partners, maintaining regulatory compliance and contributing to improved scheduling efficiency in a dynamic team environment.

Qualifications

• Experience in intake/scheduling in home health or hospice.
• Proficient with Kinnser/WellSky EMR systems.
• Familiar with home health regulatory requirements and payer authorizations.
• Excellent verbal and written communication.
• Strong organizational and multitasking abilities.
• Proficient in Microsoft Office applications.
• Medical billing and coding experience preferred.
• High attention to detail and independence in fast-paced settings.

Responsibilities

• Manage all incoming patient referrals and process intakes using Kinnser/WellSky.
• Schedule patient visits and allocate clinician resources.
• Communicate schedule changes to clinicians, referral sources, and patients.
• Verify insurance coverage and obtain required authorizations.
• Collaborate with case managers, field clinicians, and billing staff as needed.

• Track referral trends and assist in intake performance reports.
• Identify opportunities to streamline intake processes and improve efficiency.

Job Type: Full-Time / Part-Time

Position Summary:

The Intake & Scheduler oversees the entire patient intake process within a home health/hospice setting, using WellSky/Kinnser EMR. This role ensures newly referred patients are scheduled accurately, insurance authorizations are complete, and transition seamlessly into care services while supporting high-quality communication with referral partners and internal teams.

Key Responsibilities:

• Manage all incoming patient referrals and process intakes using Kinnser/WellSky
• Schedule patient visits, allocate clinician resources, handle adjustments or cancellations
• Communicate schedule changes to clinicians, referral sources, and patients
• Verify insurance coverage and obtain required authorizations
• Collaborate with case managers, field clinicians, and billing staff as needed
• Maintain compliance with state and federal home health/hospice regulations
• Track referral trends and contribute to regular intake performance reports
• Identify opportunities to streamline intake processes and improve efficiency
• Provide system support and orientation to new intake team members
