Job summary

Melmark is seeking a Med Trip Driver/Programmatic Float to support the children’s services department. The role involves transporting Melmark residents to medical appointments and maintaining supervision levels during trips, while also assisting with daily activities and documenting required data.

Qualifications

• High School diploma required; 50 college credits preferred.
• Valid US Driver’s license and ability to become Melmark-approved driver.
• 1 year experience in a related field preferred.
• Working knowledge of Windows OS and Microsoft Office.

Responsibilities

• Provides transportation and level of supervision for Melmark residents during routine and emergency medical trips.
• Ensures timely arrival to appointments and resident safety during transport.
• Secures documentation for medical trips, including referrals and insurance information.
• Obtains necessary funds for transportation costs as needed.
• Documents medical trip information and returns to Health Care Services.
• Maintains LOS for assigned residents and ensures smooth handoffs when needed.
• Assist residents with activities of daily living and daily living routines.
• Collects data for ISP, BSP, Protocol Summary, and lesson plans.
• Completes daily documentation including sleep, stools, logs, and incident reports.
