Job summary

WEST MICHIGAN COMMUNITY MENTAL HEALTH SYSTEM is seeking a Care Manager for Children's Home-Based Services in Ludington to provide intensive outreach for families with children aged 0–21. You will coordinate services across multiple agencies and support in-home development and community integration.

The role requires state licensure or eligibility as a QMHP/MHP/CMHP, training in family systems or related areas, a valid driver's license, and CPR/First Aid certification.

Qualifications

• Must hold Michigan licensure or eligibility for licensure in one of the approved professions.
• Must meet Medicaid provider qualifications and CMHP/MHP designations.
• Training in family systems theory or related youth-focused practice is required.
• Must be credentialed and privileged to practice at WMCMH by the Clinical Oversight Committee.
• Must have First Aid and CPR certification.
• Must possess a valid driver’s license and provide own transportation to various locations.

Responsibilities

• Provide intensive outreach services to children aged 0–21 and their families.
• Coordinate services between WMCMH and other community programs or service providers.
• Assist with housing, finances, nutrition, medical access, transportation, recreation, and work-related issues.
• Coordinate services between schools, DHHS, Juvenile Court, and other agencies to meet family needs.
• Travel to Lake, Mason, and Oceana counties as needed.
