Job summary

Position: Care Manager (Union) Position code: 26-39 Location: All Sites Work Hours: 40 HRS/WK Compensation Range: $24.41/HR to $31.43/HR Annual Salary: $50,768.19 to $65,380.60 Position Dates: ASAP Supervisor: Clinical Supervisor

Summary

This position is responsible for all functions of case management including advocacy, coordination, assessment, linking, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness. Settings include the CMH office, home, and/or the community.

Essential Duties and Responsibilities
  • Gather and review medical, psychological, and social history to determine client needs and capabilities toward a least restrictive environment.
  • Initiate and coordinate professional assessments, share and present assessment/treatment data as authorized and required for coordination with agencies, courts, and caretakers.
  • Complete the person‑centered plan of service, periodic reviews of progress, and assure implementation of goals in residential, employment, education, and other settings.
  • Serve as primary advocate for clients and primary liaison with families, guardians, caretakers, and involved agencies.
  • Provide consultation, referral, and in‑services to staff, caretakers, families, and agencies as needed.
  • Participate in placement planning, complete necessary forms, and coordinate residential treatment plans and visits.
  • Conduct community outreach to individuals with a history of hospital recidivism to prevent crisis situations.
  • Participate in after‑hours on‑call, crisis intervention, and other duties as assigned.
  • Participate in evidence‑based practices such as DBT/IDDT/FPE/IPS.
  • Provide care management services to veterans and their families as assigned.
  • Complete OBRA assessments and provide OBRA coordination as directed by supervisor.
  • Assist with grant requirements and ensure integrated health care needs are addressed.
  • Coordinate and link to external services as a CCBHC Care Manager.
  • Perform initial intakes as assigned by supervisor.
  • Perform other duties as assigned.
Children’s Care Manager
  • Link and coordinate services such as school, probation, DHHS.
  • Provide family psychoeducation.
  • Participate in the BTC committee as needed for individuals.
  • Work with populations 0‑21 years of age with SED or IDD Dx.
Adult Care Manager
  • Link and coordinate services such as SS, probation, DHHS, MRS, jail services, etc.
  • Provide family psychoeducation.
  • Participate in the BTC committee as needed for individuals.
Behavioralist Care Manager
  • Link and coordinate services.
  • Provide behavioral intervention support to Residential Home system.
  • Complete behavioral assessment, FBA, and behavioral plan in accordance with BTC regulations.
  • Set up data collection methods for providers to monitor progress.
  • Provide summaries or other means of communication to providers, treatment team members, and BTC on progress related to data measures.

Qualifications Requirements
  • Bachelor’s Degree in Human Services from an accredited school.
  • Preferred licensure: RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, or LPC. SST licensure for Medicaid compliance. BCaBA required for Care Manager‑Behavioralist.
  • Qualification as QMHP, QIDP, or CMHP where appropriate for population served.
  • Veteran status required when working with Veterans; for children work, 40 hours of Children’s training per year and CAFAS rater certification.
  • Minimum three years’ experience with examination, evaluation, and treatment of SED children and families.
  • Substance license preferred; encouraged to pursue MCBAP development plan.
Language Skills

Read, analyze, and interpret business periodicals, professional journals, technical procedures, and regulations; write reports, business correspondence, and procedure manuals; present information effectively to managers, clients, customers, and the public.

Mathematical Skills

Add, subtract, multiply, and divide using whole numbers, fractions, and decimals; compute rates, ratios, and percentages; interpret bar graphs.

Reasoning Ability

Apply common sense to carry out instructions in written, oral, or diagram form; handle problems involving multiple concrete variables in standardized situations.

Computer Skills

Knowledge of word processing software; recommended typing speed of 45 wpm.

Certificates, Licenses, Registrations

Valid chauffeur’s license if transporting individuals receiving services.

Physical Demands

Regularly sit, talk, and hear; occasionally stand, walk, handle objects, and lift up to 25 pounds. Vision requirements include close, distance, color, peripheral, and depth perception.

Work Environment

Moderate noise level; may travel to outside meetings or homes; exposure to outside weather conditions.
