WEST MICHIGAN COMMUNITY MENTAL HEALTH SYSTEM is seeking a Care Manager for Children's Home-Based Services in Ludington to provide intensive outreach for families with children aged 0–21. You will coordinate services across multiple agencies and support in-home development and community integration.

The role requires state licensure or eligibility as a QMHP/MHP/CMHP, training in family systems or related areas, a valid driver's license, and CPR/First Aid certification.

Pay Range: $24.52 - $30.98

The Children’s Home-Based Services Care Manager is responsible for providing intensive outreach services. The employee in this position is responsible for consulting and coordinating services, as a case management function, with other community mental health programs and/or community organization/service providers. Services will be provided in Lake, Mason, and Oceana counties on and off the program site. The incumbent may be assigned agency consumers who are not open to the Home-Based team based on capacity.

The Children’s Home-Based Services Care Manager works with families having children between the ages of 0 – 21 years old. These children will require extensive support, monitoring and follow up in order to remain in their own homes, achieve age-appropriate developmental goals, and to become integrated into normal community activities. The position requires the ability to provide/teach basic problem solving and social skills, which would help consumers to deal with difficult behavior in the home, and to be more effective at finding and using natural, community supports. This includes assisting with some or all of the following: housing, financial problems, nutrition, accessing medical care, transportation, recreation, and work-related issues. Additionally, the position incumbent will coordinate services between various agencies (i.e., schools, Department of Health and Human Services (DHHS), Juvenile Court, etc.) to meet the needs of children and families.

Position Requirements

• State of Michigan licensure, certification, or registration as required by one of the following professions: psychologist (LP, LLP, TLLP), physician (MD or DO), educator from an accredited program*, social worker (LMSW, LLMSW, LBSW, LLBSW), physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed professional counselor, licensed marriage and family therapist, or a licensed physician’s assistant. If not currently licensed, certified, or registered in one of these professions, the individual must be eligible and willing to apply for and obtain such licensure, certification, or registration immediately per the guidelines for the profession. (*Educators are not required to retain their teaching certification in order to be considered qualified for their position.)
• Must meet the Medicaid provider qualifications for services for the specific program; be a Qualified Mental Health Professional (QMHP), Qualified Intellectual Disability Professional (QIDP), Mental Health Professional (MHP) and a Children’s Mental Health Professional (CMHP) or must be eligible to obtain the designation.
• Training and practicum experience in family systems theory, family therapy, child development, or equivalent experience with youth/or adolescents is required.
• Must be credentialed and privileged to practice at WMCMH by the Executive Committee of the Clinical Oversight Committee of WMCMH.
• Must be certified in First Aid and CPR.
• Must possess a valid driver’s license and provide own transportation to and from meetings and activities at varying work locations including all agency locations in Lake, Mason, and Oceana Counties.
• Lived experiences with mental illness/developmental disabilities/substance use disorders are valued.
