Job summary

Strive Health is seeking a Patient Navigator who will support our care team and keep patients engaged in our kidney care program. Most interactions occur by phone, with opportunities for in-person visits at offices or patient homes.

You will build strong relationships, educate patients about enrollment, and help navigate questions. You’ll travel within the assigned territory, meet with nephrologists and staff, and maintain CRM data to ensure smooth program delivery.

Qualifications

• High school diploma required.
• 4+ years’ experience in customer/patient-facing roles.
• Experience promoting and selling services to end users.
• Willingness to travel up to 80%, including overnights.
• Driver’s license and vehicle required for home visits within 100 miles.

Responsibilities

• Outreach to patients through face-to-face activities and outbound calls to enroll them in care services.
• Build trusting relationships and educate on enrollment requirements.
• Meet or exceed daily outreach and enrollment targets.
• Present Strive’s care model as an empathetic resource for patients.
• Maintain CRM data and patient relationships for retention.
