Job summary

Healthmap Solutions is seeking a Clinical Care Manager to develop relationships with physicians and Healthmap members enrolled in Kidney Management programs. The role focuses on coordinating care, improving health outcomes, and ensuring NCQA-aligned documentation and reporting.

The ideal candidate holds an RN license (active, unrestricted) with 3+ years in healthcare operations or care management, bilingual in English and Spanish, and willingness to travel within the assigned region, including

Qualifications

• RN license required
• 3+ years in healthcare services, clinical operations, quality, or care management
• 3 years in care gap closure or care coordination in outpatient or hospital settings preferred
• Proven ability to build and influence relationships with providers or patients
• On Island residency and regular travel within assigned region

Responsibilities

• Act as liaison between Healthmap, provider practices and Healthmap members to ensure engagement and performance with programs
• Develop provider/partner relationships to improve engagement and manage caseload
• Identify opportunities to improve health outcomes based on provider data
• Educate on best practices, care workflows, and NCQA measures
• Educate members on kidney health and related conditions
• Coordinate care navigation and outreach to engage members in the program
• Ensure timely delivery of reports to stakeholders
• Maintain documentation in compliance with HIPAA and regulations
• Travel within assigned region as needed
• Perform other related duties as assigned
