Strive Health is seeking a Clinical Lead Nurse Practitioner to advance care for patients with CHF, CKD/ESKD and other chronic conditions. You will operate across home visits and telehealth, applying advanced analytics and interdisciplinary collaboration.

The role emphasizes reducing hospitalizations, guiding transitions, and delivering patient-centered, whole-person care. Hybrid-remote work with field travel within the region, and a comprehensive benefits package including a 10% target bonus.

Qualifications

• Master's degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP) or similar with cardiology (CHF) or nephrology (CKD).
• Current state licensure; NB certification; DEA license or eligible.

Responsabilités

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs exam, orders tests and formulates care plans.
• Documents diagnoses for risk adjustment and addresses care gaps.
• Provides in-person and telehealth visits across settings.
