Job summary

Intermountain Health is seeking a dedicated RN Case Manager to coordinate specialty care across clinics and the Intermountain system in Saint George, UT.

You will assess, plan, educate, and advocate for patients, collaborating with families, physicians, and care teams to create effective plans and promote shared decision making.

The role requires RN license, BLS, 5 years direct patient care, and BSN within 3 years; advanced degrees and leadership experience are preferred.

Qualifications

• Current Registered Nurse license in the state in which you practice.
• Basic Life Support (BLS).
• Five years clinical (direct patient care) experience.
• BSN within three years of hire or promotion.

Responsibilities

• Apply the Case Management Standards of Practice (Assess, Plan, Facilitate and Advocate) to manage care provided for assigned patient population.
• Assess patient's current and future needs by evaluating patient/family understanding, collaborating with physicians and team members, and reviewing clinical information to identify barriers to care.
• Plan with patient/family and health care team to develop a treatment plan enhancing patient outcomes.
• Facilitate by promoting communication between family and team members.
• Advocate by resolving barriers to care using available resources and processes.
• Work with physicians, patient/family, and team to develop goals and interventions ensuring seamless patient care and coordination.
