Job summary

Intermountain Health St George Regional Hospital is seeking a Registered Nurse Case Manager to coordinate care across clinics and the Intermountain system in Saint George, Utah. You will assess patient needs, plan with families and physicians, and advocate for resources to reach health goals.

This role requires an RN license, BLS, and 5 years of direct patient care with BSN within three years of hire. Salary is hourly with a comprehensive benefits package and a 40-hour work week.

Qualifications

• Current Registered Nurse license in the state where you practice.
• Basic Life Support (BLS) required.
• Five years of direct patient care experience.
• RNs hired or promoted into this role must obtain BSN within three years of hire or promotion (BSN not required for Nevada positions).

Responsibilities

• Apply Case Management Standards of Practice to manage care for the patient population.
• Assess and plan with patient/family to develop and implement a plan of care.
• Coordinate communication among patients, family, and care team.
• Document interactions and interventions in the patient record and participate in team conferences.
