Job summary

Delta View Post Acute in Antioch, CA seeks a dedicated Kaiser Liaison - LVN for a full-time, hourly position. The LVN will coordinate nursing-related services, admissions, transfers, and discharge planning while coordinating with Kaiser Permanente and our clinical team.

Qualifications

• Active California LVN license required.
• Experience in skilled nursing facility (SNF) care coordination or discharge planning preferred.
• Experience coordinating with Kaiser Permanente or Kaiser teams is highly desirable.

Responsibilities

• Serve as a primary point of contact between Kaiser Permanente and Delta View Post Acute regarding resident care coordination and clinical communication.
• Coordinate admissions, transfers, and discharges to support timely transitions of care.
• Communicate with Kaiser case managers, physicians, nurses, and other healthcare professionals about resident needs and treatment plans.
• Maintain accurate clinical documentation and records, ensuring HIPAA compliance.
