Job summary

Bay Area Community Resources (BACR) seeks a Junior Case Manager, Housing- Workforce Development and Reentry, to provide wrap-around case management and housing coordination for families and TAY in Alameda County. You will connect clients to employment services, CBT referrals, and stabilization resources while coordinating with landlords and providers.

The role involves regular case conferencing with Housing Navigators and extending program reach through additional projects to improve housing

General Overview: Provide wrap-around case management services and stable housing services for families/transitional aged youth (TAY) between the ages of 18-24 who identify as being marginalized, at risk, system involved, violence involved, and/or in crisis as referred by the Dept of Violence Prevention for the City of Oakland.

Position Overview

Provide high quality case management services, including but not limited to intake, case planning, housing case coordination with Oakland Housing Authority and other Housing placement services, connection to employment services to increase income/improve credit, cognitive behavioral therapy referrals and linkage to stabilization services for victims of violence. Additionally, leading case conferencing sessions with Housing Navigator to ensure favorable outcomes for long term housing placement. As well as facilitating additional projects that increase the outcome of housing placement acceptance and/or benefit clients.

The Department of Violence Prevention applies a public health approach to violence prevention focused on community-led intervention strategies to realize sustained safety and stability of the families and communities most impacted by violence.

Junior Case Manager will be responsible for case managing all who are referred as participants to BACR’s Housing Program in Alameda County. Caseload not to exceed 10 families.

Required Qualifications

• At least one year (1) of experience in housing navigation or case management
• Working familiarity with trauma informed care and engaging with individuals that are violence involved or have experienced trauma related to violence in Alameda County
• Must have excellent oral and written communication skills, familiar with Excel
• Experience with immigration laws and policies and its impact on marginalized communities and how it affects housing
• Experience working with undocumented and/or justice system impacted youth
• Must be familiar with restorative justice, trauma informed care, and motivational interviewing
• Ability to maintain confidentiality and demonstrate a high degree of integrity
• Must have valid California Driver’s License and insured, registered and operational vehicle to use for work purposes
• Must pass a criminal background check and TB test clearance
• Availability to work in the evenings to meet with participants.

Duties and Responsibilities

• Conduct various assessments of families/ TAY to identify and address wraparound barriers, behaviors and corresponding characteristics that hinder stability and functionality
• Provide compassionate and person-centered support to individuals or families facing housing instability. Coordinate with clients, Case Managers and Housing Specialists to understand client housing needs, preferences, and any barriers preventing them from securing stable housing.
• Collaborate with Housing Navigator and social service agencies, housing providers, and community partners to streamline housing processes and enhance housing options for clients.
• Generate thorough individual service plans consisting of supportive services, resources, referrals and linkage to promote stability and functionality
• Coordinate with Finance and Manager to purchase supportive services, timely rent/deposits to landlords and complete monthly expense reports/ update trackers that correspond with rents/deposits made
• Develop relationships and maintain communication with support services network, funders, housing providers and families in efforts to case conference, strategize and better serve participants
• Data Tracking and Reporting: Maintain accurate, organized landlord records and documentation on housing availability. Comply with all data reporting requirements as set by the organization.
• Continued Education: Stay informed about changes in housing policies, regulations, and resources to provide clients with the most up-to-date information.
• Meet with participants on a weekly to monthly basis depending on the need to formally monitor progress or regression of individual service plan and implement motivational techniques to keep them engaged and ensure successful placement post service period
• Work with families in setting and reaching self identified financial, professional and personal long and short term goals to ensure housing security and increase odds of application acceptance
• Represent BACR at funder partnership meetings and events
• Attend regular staff meetings for case conferencing and/or programmatic discussions
• Commitment to professional development by attending trainings and workshops to better serve participants
• Other duties as assigned
