Position SummaryThe Jr Client Coordinator/AHCCCS Field Specialist (JRCC) is a hybrid field and operational role responsible for ensuring compliance, care quality, and service continuity across Medicaid (ALTCS), Private Duty, and VA.This role blends Field Compliance Officer responsibilities with Client Care Coordination functions, serving as the first line of visibility into client services while also supporting staffing, onboarding, assessments, and ongoing case management.The JRCC will manage a high volume caseload, conduct required monthly client visits, perform Private Duty and VA assessments, support staffing coverage, and maintain compliance-ready documentation. This individual plays a key role in safeguarding service delivery standards, supporting agency growth, and maintaining strong relationships with clients, caregivers, and referral partners.Core Functional AreasField Compliance & Monthly Visits Client Assessments (Private Duty & VA) Staffing Oversight (Private Duty & VA) Documentation, Reporting & Audit Readiness1. Field Compliance & Monthly Client VisitsVisit RequirementsConduct monthly visits for all active Medicaid (ALTCS) clients in compliance with Arizona state requirementsFollow structured visit rotation: Work with lead to determine when visits are due.Schedule visits by geographic zones/zip codes to maximize efficiencyConduct visits & assessments Tuesday–FridayOn-Site Compliance ResponsibilitiesVerify services are delivered according to the authorized plan of careEvaluate:Caregiver task completion and professionalismClient safety, satisfaction, and unmet needsIdentify:Service gapsEnvironmental risksChanges in client conditionBuild meaningful client relationships and provide supportive engagementImmediate Escalation RequirementsReport in real time:Hospital admissionsSurgeries or major condition changesSafety risks or service failuresIdentify need for additional services (Home Health, Hospice, etc.) and coordinate referrals2. Client Assessments (Private Duty & VA)Conduct initial and ongoing client assessmentsEvaluate:ADLs / IADLsMobility and DME usageCognitive and communication abilityHygiene, feeding, and toileting needsHome safety conditions3. Care Coordination & Case Management (Private Duty & VA )Support full lifecycle management of client cases, including:Assessment and staffingService plan developmentOngoing monitoringMonitor:Client condition changesCaregiver performanceService effectivenessAdvocate for clients across:Community partnersHealthcare providers4. Staffing & Caregiver OversightSupport staffing coordination and coverage managementAssist with:Filling open shiftsMatching caregivers to client needsSupporting high-risk or hard-to-staff casesMonitor caregiver performance in the field and provide feedbackParticipate in:Caregiver introductionsReassignments when necessaryStep in to support coverage or urgent operational needs when required5. Administrative & Compliance OperationsMandatory Administrative DayMonday (in-office required) for:Weekly planning and schedulingClient outreach and visit confirmationDocumentation preparation and backlog completionOperational Support DutiesUpload and maintain compliance documentation (Visit notes)Answer and triage incoming calls ( from your direct population)Assist with internal coordination and communicationsSupport special projects and internal initiativesScreening & Intake ResponsibilitiesCollect critical client information:Mobility and equipment needsHygiene and bathing setupFeeding abilityCommunication levelToileting needsEvaluate staffing feasibility and case viabilityAlign new cases with caregiver availability and skill sets6.
