Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team. Work closely with our nursing staff to provide hands-on care that brings comfort and dignity to patients at the end of life.

Schedule is Per Diem, As Needed 7 am - 3:30 pm, with coverage area in McHenry & Lake County. We offer comprehensive benefits including health, dental, vision insurance, PTO, holidays, tuition reimbursement, wellness programs, and 401(k) matching.

Qualifications

• High School Diploma or GED is required.
• Current unencumbered CNA certificate in state is required.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state-mandated is required.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide personalized care following the care plan to ensure comfort and well-being of patients.
• Assist with personal care including bathing, oral hygiene, bedding changes, clothing, and shampooing.
• Deliver exceptional customer service and support to patients and families.
• Support families with guidance and information about patient care.
• Maintain accurate documentation in home and agency records.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$22.00 - $23.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
