Job summary

Crescent Hospice in Rock Hill, SC is seeking a dedicated Certified Nursing Assistant to join our per diem hospice team. You will provide compassionate, in-home care to patients at the end of life, supporting comfort, dignity, and emotional well-being under the guidance of our nursing staff.

Responsibilities include personalized care per plan, assisting with personal care, documentation, and family support while maintaining professional conduct and adherence to regulatory standards.

Qualifications

• High school diploma or GED required.
• Holds current unencumbered CNA certificate in the state.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state-mandated.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personalized care per the plan of care and promptly attend to patient needs.
• Assist with personal care, including bathing, oral hygiene, bedding, clothing, and grooming.
• Deliver exceptional customer service and uphold professional standards.
• Support families by sharing knowledge and guidance during end-of-life care.
• Maintain accurate documentation in patient records and comply with agency regulations.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state-mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$15.00 - $19.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
