Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team, providing hands-on care that supports dignity and comfort for patients at end of life. Schedule: Per Diem, As Needed 7 am - 3:30 pm.

Coverage area includes McHenry & Lake County communities such as Crystal Lake, McHenry, Algonquin, Lake Zurich and Fox Lake. Responsibilities include following the care plan, assisting with personal care, supporting families, and keeping accurate documentation.

Qualifications

  • High School Diploma or GED required.
  • Current unencumbered CNA certificate in the state.
  • Minimum one year experience as a CNA in an acute care or long-term care setting; hospice experience a plus.
  • Possess and maintains CPR certification if state-mandated.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

  • Provide Personalized Care: Follow the care plan to ensure comfort and well-being of patients and attend to requests from patients and family/caregiver.
  • Assist with Personal Care: Support with bathing, oral hygiene, bedding, clothing, and shampooing to maintain dignity.
  • Be Dedicated to Customer Service: Deliver exceptional care and satisfaction.
  • Provide Support for Families: Guide and support families in caring for terminally ill loved ones.
  • Maintain Accurate Documentation: Record details per care plans and regulatory standards.

Are you a caring and dedicated Certified Nursing Assistant looking to make a meaningful impact? Sanctuary Hospiceis seeking a Per Diem Hospice Aide (CNA) to join our compassionate team, providing hands‑on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Schedule: Per Diem, As Needed 7 am - 3:30 pm

Responsibilities
  • Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
  • Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.

  • Maintain Accurate Documentation: Every detail is crucial in delivering high‑quality care, both in the patient's home and within our agency and regulatory standards. Your attention to detail is vital to our success.
Qualifications
  • High School Diploma or G.E.D. required.
  • Holds current unencumbered certificate in the state working as a C.N.A.
  • Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
  • Possess and maintains current CPR certification if state‑mandated
  • Current driver's license, vehicle insurance, and access to dependable transportation.

$21.00 - $23.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
