Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to join our dedicated team in Westchester. The role focuses on hands-on care, comfort, and dignity for patients at the end of life, working closely with our nursing staff.

The ideal candidate is a Certified Nursing Assistant with at least 1 year of experience in acute or long-term care, holds CPR if required, and can provide reliable transportation. We offer a supportive environment and growth opportunities within Sanctuary Hospice.

Qualifications

• CNA certification required and active in the state.
• Minimum 1 year CNA experience in acute care or long-term care.
• Current CPR certification if mandated by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide Personalized Care according to the plan of care to ensure patient comfort and dignity.
• Assist with personal care tasks such as bathing, oral hygiene, changing bedding, clothing, and grooming.
• Demonstrate exceptional customer service and compassionate care to patients and families.
• Support families by sharing knowledge and reassuring them during the patient's end‑of‑life journey.
• Maintain accurate and timely documentation in home care records and agency systems.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$21.00 - $23.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
