Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) in Westchester to deliver compassionate, patient‑centered end‑of‑life care under the nursing team. You will follow a care plan, assist with daily personal care, and help patients maintain dignity and comfort in their own homes.

The role emphasizes strong communication with families, precise documentation, and a commitment to quality care while offering a supportive, development‑oriented work environment with comprehensive benefits.

Qualifications

• High School Diploma or GED required.
• Current unencumbered CNA certificate in the state.
• Minimum 1 year CNA experience in acute or long-term care; Hospice experience a plus.
• CPR certification if state mandated.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide personalized care following the plan of care and address patient and family needs with compassion.
• Assist with personal care tasks such as bathing, oral hygiene, bedding, clothing, and grooming.
• Deliver exceptional customer service and ensure patient satisfaction.
• Support families by sharing knowledge and guidance for terminally ill loved ones.
• Maintain accurate documentation per patient, home, agency, and regulatory standards.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$21.00 - $23.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
