Job summary

Frazer Center in Atlanta seeks a dedicated Job Coach to support participants with disabilities in securing employment. Working under supervision, you will provide job coaching, transportation training, and on-site support to help individuals transition to competitive work.

You will collaborate with Employment Services and VR teams, guide interview and application processes, maintain accurate records, and help develop natural supports at the job site.

Qualifications

  • High School Diploma or GED required.
  • AA/BA in social work, special education, rehabilitation, or related fields preferred.
  • ACRE or CESP Certification; experience in job training, coaching, or placement preferred.

Responsibilities

  • Provide needed support to ensure successful job placement, with guidance from supervisor.
  • Coordinate work for SE and VR clients as prescribed by ISP and/or VR.
  • Assist participants with interviewing and application processes.
  • Provide transportation training, job training, and job coaching services.
  • Teach functional skills and work-related skills (ADL, hygiene, communication, etc.).
  • Complete required paperwork accurately and timely.
  • Maintain participant files and billing information according to regulation/policy.
  • Respond promptly to incidents per agency regulations/policy.

Job description

Enhance the job seeking skills, employment, skill attainment, and work opportunities for persons with disabilities. Provide watchful care, oversight and assistance as needed; responsible for the job development of assigned participants; responsible for the training and follow along of participants placed in community employment; communicate pertinent information to the appropriate personnel and respond to inquiries or requests for information.

PRIMARY DUTY:

With the assistance of the Lead Job Coach or Employment Services Coordinator, provides needed support to ensure successful job placement

MAJOR DUTIES AND RESPONSIBILITIES:

Coordinate work for SE and VR clients as prescribed by ISP and/or VR

Assists persons served with vocational choices and with the interviewing process and application process

Provides transportation training, job training, and job coaching services as needed

Identify and teach the appropriate functional skills, ADL skills, and work skills as needed

Teach other instructional time and rehabilitation curriculum (e.g. hygiene, food preparation, communication, simulated work, etc.)

Completes required paperwork in a timely and accurate manner, according to supervisor

Communicates necessary information to Employment Specialist in a timely manner (i.e., information regarding job placements and contacts)

Works with the participant and employer to develop natural supports at the job site

Maintain participant files according to regulation/policy

Maintain billing information according to regulation/policy

Responsible for promptly responding to and reporting to the appropriate personnel, incidents or evidence of participant abuse; critical incidents; violation of participants’ rights and other events based on regulations or agency policies

Must provide medical assistance to participants served, such as administering medication, First Aid, CPR, and/or Heimlich maneuver as trained by the agency

Assist immediate supervisor with tasks to support program and/or department operations as assigned; may also be assigned special projects

May be called support to support participants in the CAG or CAI programs when needed or necessary

May need to attend to personal support needs of participants, which may include assistance with transferring, ambulation, communication, or other needs in keeping with the participant’s support needed to transition to successful employment.

Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned based on the needs of participants served

Complete 16 hours of annual training as required by regulation; attend additional training as required by the program/department

Other duties as assigned

EDUCATION/TRAINING:

High School Diploma or GED required.

AA/BA in social work, special education, rehabilitation, or related fields preferred.

One year of experience working with individuals experiencing Intellectual Disabilities

ACRE or CESP Certification, experience in job training, coaching, job placement preferred.

Must meet requirements based on a motor vehicle report that states no more than two chargeable accidents, moving violations, or any DUIs in a three (3) year period within the last five (5) years of the seven (7) year Motor Vehicle Record (MVR) period.

PHYSICAL REQUIREMENTS:

Must be able to lift, push, and pull a minimum of 50 pounds

Ability to reach, bend, and stand for long periods of time

Ability to lift from floor to chest level

May occasionally life, push, and pull up to 100 pounds
