Qualifications

• Bachelor's degree in social work or related field required.
• At least one year of experience in the social services field; case management experience preferred.
• Familiarity with medical/MH terminology and ability to access community services; licensing preferred, not required.

Responsibilities

• Provide discharge planning for patients in a correctional environment and arrange community resources.
• Set up community appointments and assess patient financial resources.
• Document discharge plans in medical records with timely final documentation.
• Collaborate with a multidisciplinary team and maintain relationships with community resources.
• Participate in client satisfaction surveys and continuing education; attend required trainings.
