Job summary

Community Bridges, Inc. (CBI) is seeking a Jail Coordinator to deliver treatment and discharge planning for County Jail Program Patients. The role includes aiding inmates with release planning and coordinating access to ongoing behavioral health services.

The position requires prior experience in criminal justice or health settings, a high school diploma or GED, and the ability to pass a background check. Training and support are provided to ensure quality care coordination.

Qualifications

• Highschool diploma or GED required. Associates degree or higher preferred.
• Experience in a criminal justice setting, behavioral health, physical health or substance abuse setting required.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Fingerprint Clearance Card preferred.
• Must be able to pass a background check and receive clearance to go into the County Jail.

Responsibilities

Job Summary

The Community Bridges Inc. (CBI) Jail Coordinator is responsible for delivering treatment, and discharge planning services to identify County Jail Program Patients. The Jail Coordinator is responsible for assisting inmates with jail release planning and coordinating into behavioral health services. The Jail Coordinator will participate in a weekly staffing with probation and jail staff to provide updates on each patient and coordinate care.
