The Community Bridges Inc. (CBI) Jail Coordinator is responsible for delivering treatment, and discharge planning services to identify County Jail Program Patients. The Jail Coordinator is responsible for assisting inmates with jail release planning and coordinating into behavioral health services. The Jail Coordinator will participate in a weekly staffing with probation and jail staff to provide updates on each patient and coordinate care.
