Job summary

COMHAR INC. is seeking a skilled Program Specialist to coordinate and implement Individual Support Plans (ISPs) for individuals with intellectual, developmental, and behavioral health needs in the Philadelphia area.

You will work with participants, families, and staff to ensure services meet needs, promote independence, and support dignity, well-being, and community integration, with a 8:30 AM to 4:30 PM schedule and no weekends on-call.

Qualifications

• Associates or 60 college credits with 3+ years related experience in ID required.
• Bachelor's degree in Human Services, Social Work, Psychology, or related field preferred.

Responsibilities

• Coordinate and complete initial, annual, and revision assessments for assigned caseload.
• Identify individualized consumer support needs for direct supervision or unsupervised periods.
• Provide ISP-related training to Direct Support Professionals (DSPs).
• Develop, review, and revise ISPs, ISP Annual Updates, and all ISP revisions.
• Attend ISP meetings and serve as Plan Lead when needed.
• Ensure compliance with DPW 6400 regulations for medication management, site safety, and training.
