Job summary

Lifepoint Health seeks an Outcomes Manager for Inpatient Rehabilitation at Baptist Memorial Rehabilitation Hospital in Germantown, TN. This full-time role runs Monday–Friday, 8:00 a.m.–4:30 p.m., coordinating IRF-PAI data collection with the interdisciplinary team.

You will ensure quality indicators documentation, complete admissions and discharges within CMS timelines, and abstract necessary information from medical records for accurate QI scoring. Team collaboration is essential.

Qualifications

• Must hold a clinical degree and current license to practice (OT, PT, RN, SLP, or MSW).
• Experience with quality indicators and CMS/IRF processes is preferred.
• Strong data entry and documentation skills are required.

Responsibilities

• Coordinate IRF-PAI data collection with the interdisciplinary team.
• Ensure complete documentation for QIs in the medical record.
• Complete admission and discharge IRF-PAIs within CMS timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Ensure admission IRF-PAI within four days of admission and discharge IRF-PAI within five days of discharge.
• Enter data into CMS-accepted software within designated timeframes.
• Maintain and update the IRF-PAI log and verify accuracy against patient location roster.
• Assist with education on QIs and documentation; support team competency development.
• Collaborate with nursing, therapy, case management, and medical staff for accurate QI scoring.
